Notes 1. Added fields 'Phone Number', 'Name' and 'Company Name' to the email form if the user belongs to 'Registered' group.
2. Pre filled the new form fields when the user is logged in as registered user.
3. Changed the email format for the registered users.
4. Removed the header image and footer link on the email.
